Driving Directions

Exit I-10 at Live Oak Canyon/ Oak Glen Road and go north. Turn right at Yucaipa Blvd, then turn left at the first signal into the Yucaipa Valley Center. Turn right as you enter the parking lot. Located on the Southwest side of the lot.

The beautiful Yucaipa Valley office opened in January 2008 in the Yucaipa Valley Center. An on-site Patient Representative is available to provide an orientation, answer questions about the services and staff, help patients choose a physician, and to handle billing questions.
