Medical Services: Call the operator at (909) 793-3311 or any facility number and ask for your doctor’s office for assistance with your medical needs including refilling a medication, authorizations, medical concerns, etc.

New Patients/General Help: Speak to a Patient Service Advisor if you would like to learn more about Beaver Medical Group and need to select a new physician. They can also help with many of your general questions (not including medical advice).

Concerns: If you are an HMO patient, call the member services line on the back of your insurance card to file an official concern/grievance. The insurance company will work with Beaver Medical Group to resolve your issue.   Scroll down for information regarding reporting compliance concerns.

If you are not an HMO patient (PPO, no insurance) and have a concern, you can call the operator to be transferred to the site where your concern originated.

Reporting Compliance Concerns

If you become aware of any situation that may implicate Beaver and Optum’s high ethical or legal standards you must report the matter to the Compliance or Privacy office of your physician, medical group, or health plan. All concerns are kept confidential to the extent possible within our legal obligations and, if you choose, may be reported anonymously. Beaver Medical Group and Optum has a no-tolerance policy for retaliation or retribution against anyone who in good faith reports suspected violations.
