Hana Hawthorne LSW, ACSW

Biography:

Hana earned both her Bachelor’s Degree and Master’s Degree in Social Work at Clark Atlanta University. She is a licensed Social Worker and an associate level Clinical Social Worker. She has over 10 years of experience as a social worker. She has served lower socioeconomic populations in various capacities. Her role in ACM is to provide support to the staff, EPIC Members, EPIC Medical Groups and stakeholders. Hana and the social services team continuously search for appropriate community resources to address and identify social determinants of health.
