If you have a concern: The Utilization Management (UM) staff is available
for callers with questions about the authorization process, approvals/
denials, and/or UM criteria eight hours per day, Monday - Friday. Patients
can either call collect with these questions to (909) 793-3311 ext 2384
or toll free (877) 335-4155. All concerns are entered into our Quality
Improvement Tracking System for review. You also have the right to file
a formal concern/grievance with your Health Plan by contacting them with
the 1 (800) number on the back of your Health Plan card.
