Your new physician would like to obtain copies of your medical records from your previous physician to review your health care.

Please follow these simple steps to have copies of your medical records transferred to your new Beaver Medical Group physician.

1. Obtain a written authorization form to release medical records from your previous physician or from the Medical Records Department at Beaver Medical Group. You may download the form or call the Patient Liaison at 909-478-5112 to obtain the form.

2. Complete the authorization and mail it to your previous physician or medical group. If you have had several different physicians or medical groups, a form should be mailed to each facility.If you have any questions on how to correctly complete the form, the medical records staff at any of our medical groups can assist you with completing and mailing the authorization form to your previous health care provider.

3. Your previous physician or medical group will then send a copy of your medical records to Beaver Medical Group.
